Course Evaluations
General Information
The course/faculty evaluation website was developed at COM. The website will be used for evaluation of most courses. You will be asked to login with name and PID number. The system keeps track of individuals who have completed evaluations so it can indicate which ones you have completed. The information entered for evaluation is stored in a SEPARATE database from the name information, and there is absolutely no way to link a student name with the course evaluation data.
PBL domain electronic eveluations will be available on Wednesday after each domain exam and will be active for one week. There will be multiple sections in order to evaluate lectures and resources used in the domain. Please complete all sections.
At the end of each Preceptor period, you will be asked to complete a seperate evaluation of your PBL group preceptor.
Specific Instructions:
Click on the link at the top of this page to go to the evaluation site.
Click on the green 'Student Login' button. Enter your first name, last name and PID number, e.g. A12345678. Your name must be entered as it appears on the class list. If you cannot login, contact Dr. Lovell in the Block I office. After entering the information, click on 'verify student'.
On the next page, select either 'Course Evaluation' or 'Faculty Evaluation'.
On the next page, to evaluate jointly taught courses, select the button 'CHM YEAR II'. Click on 'submit selection'.
Click on the course for which you are providing input .
For faculty evaluations, click on the name of the faculty member you wish to evaluate. Fill out the evaluation form by clicking in the appropriate box and then by typing comments to the open-ended questions. When you are done, click 'Send Information'.
